I know it is early in the school year but I have some time right now and I'd like to find resources on how to write a good annual performance review esp. with writing Pupil Progress Indicators.

Is this similar to goal setting or report cards? What do you mean by annual performance review? Any specific skills?

I'm so sorry that you've been waiting so long for replies!
I'd look at the goals you have set for that term/semester/quarter and go from there. If you have five goals, make five sub-grouping like this:
Goal: writing name
Progrees Made: writes letters correctly and remembers how name is spelled
Work Needed: letters are not placed in a straight line nor are they sequenced correctly
Brief and concise is always best for parents.
If you meant you are reviewing collegues, that is a bit different. I'd suggest a chart. List things like cooperation with co-workers, class control, involvement with students, or whatever you're looking for. Then have a scale of one to five to rate them on. In the third column put a blank area to write a one or two sentance comment.

If you're looking for a standardized format, I've seen the following used in some school systems:
Excellent: Student is very consistent in demonstrating ALL characteristics under Behavior andWork/Study Skills. (All items under the area must receive a check before an E can begiven for the area.)
Satisfactory: Student demonstrates characteristics listed under Behavior and Work/Study Skills mostof the time.
Needs Improvement: Student is inconsistent in demonstrating characteristics listed under Behavior andWork/Study Skills.
Grades of E, S, or N are given based upon an accompanying criterion check list:
For example:
Behavior:
Follows rules
Demonstrates self-control
Handles conflicts and problems appropriately
Respects adults and peers
Demonstrates concern for feelings of others
Work/StudySkills
Works well as a group member
Works independently
Completes tasks in a reasonable length of time
Responds appropriately to directions
Demonstrates good listening skills
Shows interest in a variety of activities
Shares materials and equipment
