Do you know a birthday poem?

I taught a few years ago; took some time off to care for an ill family member and am returning to a first grade classroom this year. One school I taught at didn't allow us to sing happy birthday and I used to have a great birthday poem that let the kids put in their their name, age, and something else?? Anyway, it worked much better than singing because the birthday girl/boy got to customize it and the kids didn't get quite so silly as they do with the birthday song.

Does anyone know of a fun birthday poem for first graders? Thanks!
